How you'll receive records
We'll send electronic records to the email address and phone number you provide during intake. It's your responsibility to keep this contact information current so you don't miss anything important.
Requesting paper copies
You can request a paper copy of any electronic record, or withdraw your consent to electronic delivery, by contacting us at 213-468-6170. Withdrawing consent may slow down your case while we arrange paper delivery.

To view and retain electronic records, you'll need a device with internet access, a current web browser or email client, and the ability to save or print PDF documents.
